Regarding bikes, personally I don't ride one, but if I did the, cows in the road have no consideration for motorists, and are a nightmare when you try and get their details in the event of an accident [all the locals will side with the cow]

Malaria tablets yes or no is a personal choice, bottom line is if you get it, it can mess you up! - Although I live there 11 months of the year and don't take them - touch wood no problems yet... Also the local population also lives there without medication...
Bikes, Apply for an Internation Licence (like Dribyar said only about a fiver (just renewed mine - shame they only last a year!) I love riding the bike out there, ALTHOUGH even if you are expeinced take it slow as things do tend to pop out of nowhere - the driving conditions are VERY different... Oh you can get new bikes for 100 Rs/- (it is legal to drive them as long as you are not renting them off the owner) anyone you hit would also be covered by the bikes insurance (if it has any) if you were in an accident (If you're not renting....)
What ever you do... Be safe, there are cows, dogs, people, bikes, cars, lorries, big huge buses who will test your witts and skill... Take care
