The trouble is that it's not just Goa but endemic all over India.We have just returned from Hampi,our hotel was in Hospet.The town of Hospet was filthy smelly you name it,even at the hotel the rubbish was just thrown into the river adjacent to the hotel.Its going to take many decades to educate the people in basic rules of sanitation.

Yes Hospet is dirty, the people cook on open fires, there is a traffic jam of bullock carts with families going to work in the fields every morning, but this is typical of rural India.
