Was the booking done over the phone Caz? As if so it sounds to me as though it's their fault.

Quite a few of the airlines change spelling mistakes for free.
You could try and ask them to confirm, but charges like that are probably detailed in their Terms & Conditions
Sounds like a pretty standard name change fee. When did you book the flight/holiday? Did you check your confirmation when you received it?
